A completed set of signature pages is required as part of your application. You can download them using the link below.

This document must be signed by the applicant, their chapter adviser, their school's administrator, and a legal guardian.

The signatures confirm that all parties understand and agree to the following commitments for the 2026-27 school year:

• Oklahoma FCCLA's Code of Ethics
• Release of the applicant's transcript
• Social media release
• Attendance obligations as a State Officer

After applications are received and processed, candidates will be notified and asked to come to the interview process. Interviews will be held on January 28th-29th, 2026 at the Oklahoma Department of Career & Technology Education in Stillwater, OK.
